Leave, expenses, and onboarding already have their own screens. A custom form is for the rest: a kit request, a working-from-home note, a return-to-work conversation.

Build it, send it, keep the answer
Add the fields you need, including questions that only appear when an earlier answer says so. No separate form tool and no copy-paste back into the employee record.
Submissions stay with the person. If the answer is really a permanent fact, such as a cost centre, put that on a custom field instead of asking for it on every form.
